Witnesses who had been in contact with Mr. B prior to his hospital admission were interviewed. $Q H[DPLQDWLRQ RI 0U %·V PHGLFDO UHFRUGV VKRZHG WKDW he attended a medical examination at the time of his admission and was found to be suffering from Hepatitis & KLJK EORRG SUHVVXUH DQG EHKDYLRUDO GLVRUGHUV UHODWHG WR GUXJ XVH 5HFRUGV VKRZ WKDW IURP WKH WLPH RI DGPLVVLRQ WR WKH WLPH RI KLV GHDWK 0U %·V FRQGLWLRQ was continuously monitored at the Rehabilitation Centre Clinic and he attended appointments with a specialist psychiatrist at the clinic. 2Q 0D\ 0U % ZDV UHFRUGHG WR EH LQ DQ DJLWDWHG state and wanting to be transferred to a psychiatric KRVSLWDO $ WUDQVIHU ZDV DUUDQJHG DQG 0U % UHPDLQHG DQ LQ SDWLHQW DW WKH KRVSLWDO XQWLO 0D\ ZKHQ KH ZDV WDNHQ WR WKH 6DOPDQL\D 0HGLFDO &RPSOH[ EHFDXVH he was unwell and suffering from a high fever. Amedical UHSRUW FRPSOHWHG DW WKH 6DOPDQL\D 0HGLFDO &RPSOH[ UHFRUGHG WKDW D &RPSXWHG 7RPRJUDSK\ &7 VFDQ ZDV performed following his diagnosis with pneumonia. Mr. % ZDV WKHQ DGPLWWHG WR WKH LQWHQVLYH FDUH XQLW ZKHUH KH VXIIHUHG D FDUGLDF DUUHVW RQ 0D\ $IWHU DQ LQLWLDOO\ VXFFHVVIXO UHVXVFLWDWLRQ 0U % VXIIHUHG D IXUWKHU arrest and subsequent attempts to resuscitate failed. 7KH 2PEXGVPDQ 2IÀFH UHTXHVWHG WKH ÀOH RI WKH 3XEOLF Prosecution investigation into the death of Mr. B and the related forensic medical report. It was recorded that the death of Mr. B was due to heart and respiratory FRPSOLFDWLRQV DQG IDLOXUH GXH WR LQÁDPPDWLRQ ,W ZDV further recorded that no evidence of wrongdoing or criminal acts was found and that the death was by natural causes. +DYLQJ H[DPLQHG DOO RI WKH &&79 DQG GRFXPHQWDU\ the Ombudsman investigation found no issues of concern relating to the treatment and care of Mr. B. Investigators requested all Mr. &·V PHGLFDO UHFRUGV IURP WKH WLPH RI KLV DGPLVVLRQ WR the Reform and Rehabilitation Centre to the time of his GHDWK 7KH\ DOVR VHFXUHG 0U &·V 5HKDELOLWDWLRQ &HQWUH UHFRUGV DQG &&79 IRRWDJH VKRZLQJ 0U &·V WUDQVIHU WR WKH FHQWUH·V FOLQLF EHIRUH KH ZDV WDNHQ WR KRVSLWDO Interviews were carried out with inmates who had shared a cell with Mr. C. :KHQ LQWHUYLHZHG 0U &·V FHOOPDWHV VDLG WKDW KH ZDV in good health in the period leading up to the week EHIRUH KLV GHDWK 7KH\ VDLG KRZHYHU WKDW DURXQG D ZHHN EHIRUH KLV GHDWK 0U & WROG WKHP WKDW KH ZDV concerned about the presence of blood in his urine and the appearance of red veins on his feet. They said that he visited the Rehabilitation Centre clinic and received WUHDWPHQW WKDW LPSURYHG KLV FRQGLWLRQ 7KHQ IRXU GD\V EHIRUH 0U &·V GHDWK KLV V\PSWRPV UHFXUUHG DQG KH again visited the clinic.
